We use SurveyMonkey Apply to collect and applications. To do this we need some to store some personal data and we need your consent to do this.


It is the Norwegian foundation Stiftelsen Dam(Akersgata 28, 0158 Oslo, Norway) is managing the site and is the data controller.


Data processor is SurveyMonkey Inc., 1 Curiosity Way, San Mateo, California 94403, USA. Read their privacy policy here. SurveyMonkey is the owners of SurveyMonkey Apply. We also recommend you read SurveyMonkey Apply’s privacy policy.


The personal data described below is collected to be able to:

  • Contact applicants regarding questions related to their applications
  • Help applicants/users when they contact us with questions regarding their applications
  • Verify the existence of applicants and collaborating researchers
  • Evaluate/review the applications/proposals



Roles 

We collect personal data for different roles in the process and the research project: 

  • Primary administrator - the person signing up for an account in SurveyMonkey Apply and who managed an organization. Usually, the person submitting an application.
  • Administrator.
  • Non-administrator.

The process and forms

The personal data is provided in different stages of our application process.

  1. Registering a user.
  2. Application form. To apply you are required to fill out and submit an form and a CV for the project leader.


What personal data we collect and why

Registered user

When you register in SurveyMonkey Apply you will have to register using the following personal data:

  • Name
  • E-mail

Why we need your name and e-mail and when it will be deleted

This information is collected for us to be able to communicate with our users and to connect users with their tasks (like filling out different forms). Also, the information is used to check if we have received similar applications in previous application rounds.


Because of this we need this information as long as you are a registered user. You can ask for the personal data to be deleted at any time. Use this form to request an overview of what information we have stored and to ask for deletion.

Additional personal data collected if/when you fill out an application

When you start filling out the application form, you will be asked to provide the following information regarding the important persons in the projects: 

  • Name 
  • E-mail 
  • Phone number 
  • Workplace 
  • Title 


Why we need this information and when it will be deleted

This information is collected to enable us to verify the existence of the persons and to assess the quality of the project. Also, the information is used to check if we have received similar applications in previous application rounds.


Because of this we need this information as long as you are a registered user. You can ask for the personal data to be deleted at any time. Use this form to request an overview of what information we have stored and to ask for deletion.


Additional personal data in the uploads required to submit the pre-proposal

CV. We ask for all the above mentioned information and additional information on previous work experience (which might be personal data) in the CV that we ask all applicant to upload before submitting the application.

Why we need this information and when it will be deleted

We need this information to be able to evaluate/review the qualifications of the project leader and the project environment. Also, the information is used to check if we have received similar applications in previous application rounds and to check our grant processes for biases (for example if there are gender differences in success rates).


Because of this we need this information as long as you are a registered user. You can ask for the personal data to be deleted at any time. Use this form to request an overview of what information we have stored and to ask for deletion.


Who will get access to the personal data?

Administrative staff 

The administrative staff of Stiftelsen Dam and The Kavli Trust and The Kavli Trust Programme on Health Research will have access to all the information described above. The members of the administrative staff are presented here.

Reviewers

The reviewers reviewing the application will have access to the information in the application. The committees are presented here.

Board members

In order to make the final decisions on the applications the board members will need some information (sometimes all the information submitted in the pre-proposals and the full proposals). The Members of the Board are presented here.

How can you check what personal data we have stored?

Site users have the ability to request to see all the information available on them within the site. Users may contact the administrator directly or may submit a request through SurveyMonkey Apply’s form.[1]
